QM/MM and AddRemove model

 The AddRemove model, from Marcel Swart, has been made ready for standard usage and documented more fully in the QM/MM User's Guide. The AddRemove model is a new coupling model to connect the QM and MM regions. First results with geometry optimizations look very promising.
